1. Introduction

Bonita Medical Aesthetics & Wellness is operated by Jessica Caceres Nursing Professional Corporation ("Bonita", "we", "us"). By using our website, shop or services, you agree to these terms.

2. Our services

• Treatments are provided by regulated health professionals, including nurse practitioners and registered nurses, and by trained medical aestheticians.

• Every treatment starts with an assessment, and we may decline a treatment that is not safe or suitable for you.

• Results vary from person to person and cannot be guaranteed.

• Information on our website and social media is general education, not medical advice.

3. Appointments and cancellations

• We require at least 48 hours' notice to cancel or change an appointment.

• Late cancellations, changes and missed appointments are subject to a fee.

• Consultations are $50, credited toward your treatment. Tattoo removal consultations are free.

4. Payments and refunds

• Payment is due at the time of service. HST is added where it applies.

• Completed treatments are non-refundable.

• Packages and memberships are for the person named on them and are non-transferable. Packages do not expire.

• Gift cards do not expire and cannot be redeemed for cash.

5. Your responsibilities

Please share your full and accurate health history, tell us about any changes before each visit, and follow the pre-care and post-care instructions we give you.

6. Online shop

We ship within Ontario and offer pickup at our St. Jacobs clinic. Shipping, pickup, returns and damaged products are covered by our Shipping, Returns & Refunds policy.

7. Privacy

Your personal information is handled according to our Privacy Policy.

8. Text message terms

By opting in, you agree to receive text messages from Bonita, including appointment reminders, confirmations, post-care instructions and, if you opted in to marketing, promotions and events. Message frequency varies.

Reply STOP to any of our messages at any time to unsubscribe, and you will receive one message confirming you have been removed. To rejoin, sign up again the same way. Reply HELP for assistance, or contact info@bonitamed.com or 519-404-3444.

Message and data rates may apply. Carriers are not liable for delayed or undelivered messages.

9. Changes to these terms

We may update these terms from time to time. The date at the top shows the latest version. These terms are governed by the laws of Ontario.
